COVID-19 Wellness Behavior Survey

 

  You are invited to take part in a research study that is being conducted by Alex DelCollo, a Senior Program Coordinator, Sara Elnakib, an Assistant Professor, in the School of Environmental and Biological Sciences Cooperative Extension, Family and Community Health Sciences Department at Rutgers University. 

 The purpose of the research is to look at the health habits of people before and after COVID-19. You will be asked to answer survey questions about your health habits. The information will be anonymously collected. No one will know which responses are yours. Your participation in the study will be about 15 minutes. We do not foresee risks to subjects participating in this study. You will not be paid to take part in this study. You may receive no direct benefit from taking part in this study however, the information provided will assist the Department of Family and Community Health Sciences in addressing your current health and wellness needs.

About the Extension Foundation

The Extension Foundation was formed in 2006 by Extension Directors and Administrators. Today, the Foundation partners with Cooperative Extension through liaison roles and a formal plan of work with the Extension Committee on Organization and Policy (ECOP) to increase system capacity while providing programmatic services, and helping Extension programs scale and investigate new methods and models for implementing programs. The Foundation provides professional development to Cooperative Extension professionals and offers exclusive services to its members. In 2020 and 2021, the Extension Foundation has awarded 85% of its direct funding back to the Cooperative Extension System, 100% of funds are used to support Cooperative Extension initiatives.
